Registration Requirements

Required Materials

Please bring the following items to our central office, Lincoln Center, to complete your child's registration.

1. Proof of residency in the name of the parent or legal guardian (one of the following)
  • Current lease or rental agreement
  • Settlement statement, final sales contract or contract to purchase with occupancy date
  • Current Orange County property tax bill
  • Complete gas, water or power bill or activation dated within the past 60 days 
If these documents are not in the name of the parent or guardian, please call the Registrar at 919-967-8211, x28268 for additional information.

2. Proof of custody (legal guardians only). We require one of the following:

  • Court-ordered custody documents
  • Court-ordered guardianship documents
    Notarized statements are not acceptable.

3. Photo identification for the parent or legal guardian (one of the following):

  • Valid drivers’ license
  • Valid state photo ID
  • Valid passport
  • U.S. Military ID

4. Proof of student's age (one of the following) 

  • Original birth certificate
  • Valid passport
  • Photocopies or a mother’s copy from the hospital are not acceptable
5. Most recent report card and/or official withdrawal form from previous school (grades 1–12).


7. NC Health Assessment (Required of all students new to NC Public Schools, including rising Kindergartners).

8. High school transcript (if applicable).
